Description
After installing dsh-TUI, the DeepSeek Harness application crashes on startup and can no longer be opened. The application fails during boot with the following error:

Error: dsh: plugin tree failed to load: failed to apply loader entry include (cordis:include): duplicate loader entry id: agent-presets
TypeError: duplicate loader entry id: agent-presets
This appears to be caused by a loader entry ID collision — agent-presets is being registered twice, likely conflicting with a built-in entry in DeepSeek Harness.

OS: macOS
Node.js version: v24.18.1
DeepSeek Harness version: 0.1.0-rc.5
dsh-TUI version: 1.4.1
Steps to Reproduce
Install DeepSeek Harness
Install dsh-TUI via dsh plugin install (or dsh harness install)
Restart the application
Expected Behavior
The plugin installs successfully and the application starts normally.
Actual Behavior
The application crashes immediately on startup with the error above. The application is completely unusable until the plugin configuration is manually removed.
