Inaccurate citation counting due to insufficient deduplication of URLs from different sources.

## What happened?
When the same URL is returned both as a provider citation and written in the answer text, NiubiGEO keeps both entries because citation deduplication includes the source type in its key. The report can therefore show two citations for one underlying URL.
This is most visible with providers that return native citations while also printing source links in the answer.
## Steps to reproduce
1. Pass two citation objects to `dedupeCitations` with the same `url` but different `source` values, for example `provider_annotation` and `answer_text_url`.
2. Inspect the returned array.
3. Both citations are retained even though they point to the same page.
## Expected behavior
A canonical URL should be counted once per response. When a native provider citation and a text URL duplicate each other, keeping the native citation is sufficient because it retains the richer source metadata.
## Environment
- NiubiGEO commit: e6b9533
- OS: Windows 11
- Node.js: v25.2.1
